Lost Dividend Paper Application -- It Happens

I truly understand your point. This system is certainly azz-backwards. I've got one for you back in 1997, I filed for my dividend and my child's dividend with a witness watching cause back then you needed a couple of verifiable signatures on your dividend application, my child had just been born the prior year.

I put my application and hers in the same envelope along with her birth certificate.Even with all the witnesses statements I was told they did not receive it. I am not crazy I know it was in there. Well I went through some type of informal appeal process, but to make a long story short, it is not a perfect system and the Alaska Dividend office does lose applications -- I repeat they do lose applications.

To make a long story short , I never did receive my child's dividend even though I did everything I could possibly do on my end.
